POSITION TITLE: Digital Navigator
JOB DESCRIPTION SUMMARY: The Digital Navigator is a trusted guide who assists community members
with ongoing, individualized support for accessing affordable and appropriate connectivity, devices, and
digital skills. The Digital Navigator provides individualized or small group assistance to underserved
community members, strengthening community members’ digital knowledge and ability to navigate
technology. This includes increasing their ability to access affordable home internet service, affordable
internet-capable devices, so that they may become confident internet users and be able to fully
participate online.
Position Status: Contractual position at 14 hours/week, hourly
Pay Range: $18.00 – $23.00/hour
Grant-funded position
This position will report directly to the Adult Literacy Coordinator
Duties and Responsibilities:
● Initiate interactions with clients seeking assistance.
● Discuss with each client their home internet access or need for home internet access,
technology experiences, and their access to and use of devices.
● Assess clients’ access to technology, current digital skill level, connectivity needs, and internet
use priorities. Set agreed goals for digital navigator services.
● Advise clients about free or low-cost home internet service options for which they may qualify,
assist clients in applying for services they choose, and support their efforts to secure service.
● Provide one-on-one support with digital skills, help community members with online access and
registrations, and provide referrals to sources of additional digital skill training, to fulfill the
agreed-upon goals.
● Facilitate presentations about digital navigation to community members.
● Contribute to marketing and outreach for the digital navigator program.
● Responsible for tracking client data, including progress and satisfaction. Maintain accurate and
timely records and carry out grant updates and reporting requirements as required.
● Other duties as assigned.

Qualifications:
● High School Diploma/GED required. An Associates or Bachelor’s degree preferred.
● Experience with program development and implementation.
● Experience working with adult learners, and able to effectively engage and teach adult
learners.
● Strong digital navigation skills.
● Possess a solid ability to integrate and use technology for instructional and educational
purposes. Strong working knowledge of Microsoft Word, Excel and PowerPoint, Google Drive,
Canva, Zoom and Gmail.

POSITION TITLE: Community Health Worker Contractor
JOB DESCRIPTION SUMMARY: The Highwood Library & Community Center is an innovative anchor that enriches, empowers, educates, and embraces the community it serves. Our vision is to uplift, inform and grow the community through the library’s collections, programming and engagement strategies.
The Community Health Worker will work collaboratively with the Health Equity team and library team to strengthen community members’ health knowledge and connection to resources to members of the Highwood community. This position will work closely with team members and partner agencies to heighten awareness of health resources and increase health literacy and access to health resources to address the health needs of immigrant and marginalized populations.
Position Status: Contractual Position
Hours: 18 Hours/week, including evenings and weekends
Compensation: $17.00 – $20.00/hour depending on qualifications
Bilingual (English & Spanish) required.
This position reports to the Health Equity Manager
Responsibilities include:
PROGRAMMING
- Work directly with Health Equity Manager to develop implementation strategy of CHW efforts.
- Understand the needs of, gaps in services offered to and barriers faced by the population served and offer programming that meets adults where they are at.
- Stay educated on latest health information to provide accurate education to the community.
- Facilitate health education at weekly health workshops (Charlas de Salud.)
- Partner with organizations to host health screening events, vaccination events and health fairs.
- Responsible for the set-up/break down of events and for securing volunteers to support events.
- Assess program performance on an ongoing basis to ensure that grant outcomes will be met using Pre/Post testing.
- Track statistical data, including participant demographics, assessments, success stories and programmatic outcomes.
- Provide substitute support for library coverage as needed.
COMMUNITY ENGAGEMENT
- Conduct extensive community engagement, including information about trainings, services and resources offered by the Highwood Library & Community Center.
- Conduct regular community needs and strengths assessments to create programs and services that are relevant to the community.
- Work closely with other team members to conduct community awareness and marketing of all Library programming with emphasis on Health Equity programming.
- Other duties as assigned
STRONG PARTNERSHIP DEVELOPMENT
- Identify and foster relationships with key health care providers, organizations, and networks to expand programmatic efforts.
- Maintain accurate and current information on all program partners and participants.
- Leverage partnerships with key health service providers to carry out a variety of services to promote health and wellness in Highwood.
- Represent the library at partner organization and community planning meetings through a community health worker lens.

- Fluent in oral and written Spanish and English.
- Able to effectively understand, engage and communicate with the community.
- Strong working knowledge of Microsoft Office, including Word, Excel and Powerpoint.
- Possess a solid ability to integrate and use technology for instructional and educational purposes (Zoom, Teams, Google docs or other cloud-based technology.)
PHYSICAL DEMANDS
This position will set up and break down events including moving and setting up tables and chairs. Walking in the community to businesses and places frequented by community members to conduct outreach is required. Carrying boxes and materials of up to 30 pounds is necessary for events and presentations. POSITION TITLE: Case Management Contractor
The Highwood Library & Community Center is an innovative anchor that enriches, empowers, educates, and embraces the community it serves. Our vision is to uplift, inform and grow the community through the library’s collections, programming and engagement strategies. This exciting opportunity will position the library to provide underserved and underrepresented community members with access to health education, case management, preventative health services and counseling services.
Position Status: Contractual position at 10-18 hours/week
Hours: Flexible, including some evenings and Saturdays
Compensation: $18-$21/hour based on experience
Bilingual (English & Spanish) required
This contractual position reports to the Lead Case Manager. This is a grant-funded position.
Responsibilities include:
- Create a low barrier entry for individuals to receive social services support.
- Through outreach, direct observation, or referral, identify and engage with individuals who may benefit from social service support.
- Conduct comprehensive initial interviews and assessments to identify client needs and provide referrals to internal and external resources.
- Provide short-term case management focusing on the social determinants of health.
- Provide culturally responsive advocacy with a focus on client empowerment and evidence-based practices in providing treatment.
- Maintain current knowledge of resources available in the community to support a continuum of effective services for the clients.
- Build collaborative relationships with community organizations.
- Engage proactively with stakeholders and community members when representing the library at meetings and events.
- Maintain confidentiality, and accurate records and documentation of client interactions.
- Co-facilitate Health Equity programs.
- Complete Mandated Reporter training within the first 90 days of employment.
- Assist in completing and submitting monthly reports.
Personal qualities and qualifications include:
- High School Diploma/GED required. An Associates or Bachelor’s degree preferred.
- Self-Starter with strong solution-focused skills. Ability to work independently and manage multiple tasks and interruptions.
- Skillful to respond quickly and appropriately in times of crisis.
- Adhere to a no-wrong door policy when individuals approach the library for assistance.
- Ability to work collaboratively with team members and other community agencies.
- Familiarity with local supportive services.
- Ability to maintain confidentiality, respect privacy, client dignity, and the right to self-determination. Ability to collect and report on data for internal and external use.
- Using a social justice approach, and having a demonstrated ability to work effectively and constructively with persons from a variety of ethnic, racial, and social backgrounds.
- Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
- Strong verbal and written communications skills in both English and Spanish.
- Demonstrated time-management and organizational skills with the ability to meet deadlines.
